I will suggest she start chewing Gums ( i mean medicated gums which fights cavity and mouth odor) and secondly, she should try and use mouthwash every morning and night!!! that will definately help without visiting the dentist

Very simple tell her to be brushing her teeth every 5 minutes.

take her to a shop, but 2 toothpaste and then tell her you only needed 1 you made a mistake the give 1 to her... on your way back go on and on about how you like the smell of toothpaste and how you even used to lick it when you were younger... give her a few days, repeat the story if you have to... if shes still not getting it spell it out
