You should not be surprised that most of these doctors have private hospitals and they use Government hospitals to drive traffic to their own private hospitals, it has happened to me before, but in a way there's nothing wrong in making use of that opportunity to achieve your ambition of self employment, so long you do the right thing, by not suffering the patients you're supposed to care for at the government hospitals, in doing this the patients themselves would naturally like you and they'll come knocking at your private hospital door. IT'S THAT SIMPLE.
